· Expand on Current state of Golden Health System(1)Current state is that Golden Health homeless population costs $1400 dollars per day with extended lengths of stay due to difficult discharges which places financial strain on the organization. Upon discharge, lack of community coordination leads to poor outcomes for health improvement and prevention in this population.
· Expand on Goals of Golden Health System(2)Goal of Golden Health is to be financially viable within homeless initiative in 3 years, develop partnership to ensure increased services within the community, implement emerging and innovative technologies to improve meeting needs of underserved populations, and implement a healthcare plan to improve overall health of the homeless population.
